Third Year Funding Request*

Projects that are applying for third year funding must answer the questions below and submit this information to Michelle Zechmann by email or mail by 5:00pm on January 31, 2005. You must still fill out the 2005 Pre-application form (for a budget of one year only).

  1. Describe the specific reasons why you have not been able to financially sustain your program after the two year funding period:

  2. Name the activities/steps that show that you have made substantial progress towards financial sustainability over the longer term?

  3. What steps have you documented to show that you can reasonably expect to use a final year (your 3rd) to achieve important outcomes unachievable in a 2-year time frame?

  4. How many youth or families has the project served for the past two years? How do these numbers compare to your projections as to how many you originally expected to serve? Has there been a "drift" in numbers or types of persons being served? If so, how does this impact your request for a 3rd year of funding?

  5. Do you know the average cost per child or family served by your program? What is it? This is a rough measure of how efficient you are being with grant and other resources.

  6. Please provide specific data that exhibits the impact that the project has had on the target population in the past two years. How do these data link specifically with the project’s goals and objectives?

  7. Provide data that supports your conclusions that you’re meeting -- or have met -- your goals and objectives? You should have a program logic model or some evaluation template for indicating how your services have lead to significant outcomes that argue for 3rd year funding.

  8. Our expectation is that you will be reducing the amount of your federal request through partial sustainability efforts. What percentage of the original annual federal amount (of your current grant) will you be requesting? Please provide the actual amounts of the federal requests, as well.

  9. How do you plan to financially sustain the project after the third year? Will changes in funding levels from the Crime Commission affect your goals/objectives and how you deliver services (if so, please tell us why/how)?

  10. Please provide a letter of support from your Juvenile Crime Prevention Council Chair that describes how the project fits into the counties "plan of action" and the impact the project has made on youth.

*Funding of grants for a third year is a rare occurrence and will only happen in very extreme cases. You must be able to exhibit a concerted effort to sustain your project and have made a significant impact on your target population.
